Description of Service
Palliative Care is a specialist area that provides care for people with progressive, life-threatening illness. It is active care, aiming to anticipate problems and minimise the impact of the disease so that the patient can achieve the best possible quality of life.
The Alfred Palliative Care Service provides:
- Expert advice on symptom control
- Facilitation of smooth and timely discharge from the acute hospital through efficient links with the domiciliary support services and inpatient palliative care units
- Assistance with clarification of treatment goals, decision making and enhancement of a patient's sense of control and autonomy at the end of life
- Facilitation of care provision in the place of the patient's choice
- An active role in education at undergraduate and postgraduate level for all persons involved in end of life care.
